If you’re an existing patient, you can log into the patient portal by clicking on the SIGN IN button. If you have never logged into the Patient Portal, please call our office at 432-683-8516 and we will send you an email with credentials and instructions on how to use the portal.
What You’ll Need for Your Office Visit
We want to make your office visit a hassle-free and pleasant experience. For new patients, before you visit please complete the New Patient Registration form and print it out for your office visit. If you have any questions when filling out these forms, please do the best you can, and our staff will assist you with any questions on the day of your visit.
This will simplify the check-in process. Also, please make sure to bring the following information:
- Insurance card
- Driver’s license
- List of current medications
